Application to Provide Housing for AFO Students

2023 HOST FAMILY APPLICATION

Thank you for your interest in hosting members of the Ayrshire Fiddle Orchestra as they visit our community!

General information for host families:

Dates:

  • Tuesday, July 18, 2023 from an early evening arrival through the morning departure on Friday, July 21, 2023 (3 nights).

Transportation:

  • You are asked to provide transportation for the guests for the three days.
  • The schedule will include early morning, late afternoon, and evening activities.

Meals:

  • Please plan to provide three (3) breakfasts and two (2) evening meals.

Accommodations:

  • Same gender guests will be assigned to a host family. They may share a room with separate beds.
  • Married adult chaperones will be assigned together to a host family.

Insurance/Waivers:

  • All guests are covered by the Ayrshire Fiddle Orchestra group insurance policy and have signed a liability waiver.
  • All of the chaperones and orchestra members have had COVID vaccinations.

Host families may host the following groups:

  • 2 adult chaperones, 2 students, 3 students.
  • Student ages: 12-14; 15-16; 17-18; 18-21.

You will receive a photo and brief introduction of your students including their contact information for the opportunity to reach out to them and share a photo and introduction of your family.

Criminal history and local history background checks are required for all members of the host family home. The forms will need to be fully filled out and submitted with a copy of driver’s license/state issued ID. Juveniles must be listed with a date of birth.
